What is IT Band Syndrome?
IT Band Syndrome (Iliotibial Band Syndrome) is a common cause of outer knee pain, particularly in runners and active individuals.
The iliotibial (IT) band is a thick band of connective tissue that runs from the hip down the outside of the thigh to the knee. Pain typically occurs where the IT band interacts with the outer knee.
At our East Auckland physiotherapy clinic, IT band syndrome is often related to load management and movement control issues, rather than just “tightness” of the band itself.
Common Symptoms
Pain on the outside of the knee
Pain that worsens with running, especially downhill
Symptoms that start after a certain distance or time
Tenderness over the outer knee
Pain with repetitive bending and straightening of the knee
What Causes IT Band Syndrome?
IT band syndrome is typically caused by a combination of factors:
Training Load Errors
Sudden increases in running distance, intensity, or frequency.
Hip Muscle Weakness
Weak gluteal muscles can lead to poor control of the hip and knee.
Poor Running Mechanics
Increased hip drop or knee valgus increases stress on the lateral knee.
Repetitive Strain
Activities involving repetitive knee bending (e.g. running, cycling).
How Physiotherapy Can Help
At our East Auckland physiotherapy clinic, treatment focuses on addressing the underlying causes, not just the symptoms.
Assessment
Running and movement analysis
Strength testing
Identification of contributing factors
Treatment
Load management and activity modification
Progressive strengthening programme
Manual therapy and soft tissue techniques
Education on training and recovery
Rehabilitation
Improve hip and lower limb strength
Correct movement patterns
Gradual return to running and sport
Common Misconception
IT band pain is often thought to be caused by a “tight IT band”.
However:
The IT band cannot be significantly stretched
Foam rolling may help symptoms but does not change the structure
Strength and load management are key to long-term recovery
Simple Tips to Reduce Pain
Reduce running volume temporarily
Avoid downhill running early on
Avoid sudden increases in training load
Strengthen hip muscles
Gradually return to activity
When to See a Physiotherapist
You should seek physiotherapy if:
Pain persists despite rest
Running or exercise is limited
Symptoms return when you increase activity
Pain is affecting performance
