Hip Labral Tear, gluteal tendinopathy and bursitis Treatment in Auckland: How Physiotherapy Helped Avoid Surgery

Hip pain can be debilitating — especially when it affects your ability to walk, sleep, and stay active. Many patients are told they may need surgery, but in some cases, targeted physiotherapy can provide excellent outcomes without surgical intervention.

In this case study, we share how one of our patients with a diagnosed hip labral tear successfully returned to a pain-free, active lifestyle through physiotherapy.

Understanding Hip Labral Tears and Lateral Hip Pain

The hip labrum is a ring of cartilage that helps stabilise the hip joint. When damaged, it can lead to:

  • Persistent hip pain

  • Clicking or catching sensations

  • Reduced tolerance for walking and exercise

Often, labral tears are accompanied by other issues such as tendon overload or bursitis, making the condition more complex.

Patient Presentation: Severe Pain Limiting Daily Life

Our patient initially presented with:

  • Inability to walk more than 10 minutes

  • Significant night pain, affecting sleep

  • Constant pain throughout the day

  • Unable to continue gym exercise or swimming

She had previously seen a specialist and was advised about surgical options but preferred to explore non-surgical treatment first.

MRI Findings

The MRI revealed:

  • Tear of the left hip labrum

  • Low-grade chondral wear in the hip joint

  • Mild gluteus minimus tendinosis with partial-thickness interstitial tear

  • Minimal gluteus medius tendinosis

  • Mild trochanteric bursal oedema

These findings indicated a combination of intra-articular (joint-related) and lateral hip (tendon/bursa-related) issues.

Physiotherapy Treatment Approach

At our Auckland physiotherapy clinic, we designed a comprehensive rehabilitation programme tailored to her condition:

1. Pain Management & Load Modification

  • Education on reducing aggravating activities

  • Strategies to improve sleep and reduce night pain

2. Manual Therapy

  • Techniques to improve hip mobility

  • Reduction of muscle tension around the hip

3. Progressive Strength & Rehabilitation

  • Targeted gluteal strengthening

  • Gradual tendon loading programme

  • Functional retraining for walking and daily activities

4. Return to Activity

  • Structured progression back to:

    • Gym training

    • Walking

    • Swimming

Results: Back to a Pain-Free Lifestyle

With consistent physiotherapy and adherence to her programme, the patient achieved outstanding results:

✔️ No night pain — able to sleep comfortably✔️ Pain-free during the day✔️ Able to walk more than 1.5 hours without pain✔️ Successfully returned to gym exercise and swimming✔️ Avoided the need for surgery

Can You Avoid Surgery for a Hip Labral Tear?

Not all labral tears require surgery. Many patients respond very well to:

  • Structured physiotherapy

  • Strength and conditioning programmes

  • Load management strategies

The key is a thorough assessment and individualised treatment plan.

When Should You See a Physio for Hip Pain?

You should seek physiotherapy if you experience:
