What is an ACL Injury?
The anterior cruciate ligament (ACL) is a key stabilising ligament in the knee, responsible for controlling rotation and forward movement of the tibia.
ACL injuries commonly occur during sports that involve:
Sudden changes in direction
Pivoting or twisting movements
Jumping and landing
At our East Auckland physiotherapy clinic, we frequently manage both surgical (ACL reconstruction) and non-surgical ACL injuries.
Common Symptoms
A “pop” at the time of injury
Immediate swelling
Knee instability or giving way
Pain with walking or weight-bearing
Reduced range of motion
Do You Need Surgery?
Not all ACL injuries require surgery.
Management depends on:
Activity level and sporting demands
Degree of instability
Associated injuries (e.g. meniscus)
Some patients can successfully return to activity with structured physiotherapy alone, while others benefit from surgical reconstruction followed by rehabilitation.
ACL Rehabilitation Phases
ACL rehabilitation is a structured, progressive programme that typically takes 9–12 months for full return to sport.
Phase 1: Early Stage (0–6 weeks)
Reduce swelling and pain
Restore knee extension and range of motion
Activate quadriceps muscles
Normalise walking pattern
Phase 2: Strength & Control (6–12 weeks)
Build quadriceps and hamstring strength
Improve balance and proprioception
Introduce functional movements
Phase 3: Advanced Strength (3–6 months)
Increase strength and power
Progress to running and gym-based training
Improve movement control
Phase 4: Return to Sport (6–12+ months)
Agility and change of direction drills
Jumping and landing mechanics
Sport-specific training
Return-to-sport testing
